The goal of rehabilitation is to help an individual be as independent as possible in their everyday life and participate as fully as possible in day-to-day activities. Rehabilitation seeks to optimise well-being for patients by addressing impairments, limitations, and restrictions in many areas including mobility, vision, cognition, psychological and social functions, and are a key component of health care services. These services may be delivered in multiple settings depending on the individuals’ needs and circumstances.

A fundamental part of the continuum of care, practitioners must consider personal and environmental factors when planning their activities and be able to record and access information relevant to their patient’s rehabilitation from all parts of the healthcare ecosystem.
