In order to meet the aspirations of Institute for Healthcare Improvement’s Triple Aim for Healthcare, the management and delivery of care is increasingly moving beyond the walls of an individual institution to focus on integrated and coordinated care, built around the needs and preferences of the consumer. Digital first tools and services are increasingly becoming part of the landscape of care, and the need and success of the consumer’s outcomes center on a single, integrated and coordinated experience of their care.
Healthcare organizations need real-time visibility of the performance of their services and the services of the wider health system in order to most effectively and efficiently meet the needs of their patients. However, the data required to make this a reality is often not easily accessible. With the right approach – that includes the ability to integrate and ingest data across disparate applications, link datasets, gain evidence-based insights about populations, and to support clinical workflows across organizations – these challenges can be addressed.

- Insight: Open Health Connect provides analytics and evidence-based insights about populations and individual health.
- Inform: The Analytics Workbench component provides evidence-based insights about populations and individual health, enabling investigative intelligence on the data in the longitudinal patient record through navigable dashboards and visualizations.
- Intervene: Intervene allows clinicians to visualize an event and understand the level of intervention necessary based on clinical decision support.
