OUR APPROACH: THE CONTINUUM OF CARE

In a world with an ageing population, increasing incidence of chronic diseases and complex migration flows, the care model based on “episodes of care” is no longer applicable.

The new reality requires a shift toward a new continuum of care paradigm, with the capability to support patients, caregivers, care and social institutions to manage the level of collaboration required to deliver increasingly personalised and person-centred care.

Information Technology is central to this new model and data is the driving force behind this transformation. The adoption of digital technologies enables the flow of information across multiple settings and stakeholders to help ensure the best outcome at each stage of the continuum of care: from prevention to early detection, from diagnosis to treatment, from rehabilitation to follow-up, maintenance and end-of-life. With this integrated and data-driven approach, all stakeholder in care, including the patient, their carers, healthcare providers, and healthcare professionals work together to proactively improve health outcomes for the individual and the wider population.

Dedalus offers multiple and diverse solutions to support each stage of the continuum of care and enables the integration of every care setting, institution and system in the process. Our digital health platform is the core enabler of this integration, supporting efficient care planning, engagement and citizen empowerment.
